If you’ve finished PT after a hip labral tear and still can’t cut, sprint, or play without pain, you’re not alone. Many athletes hit a plateau after standard rehab. The truth is, traditional PT often stops short of what’s required to return to high-level sport fully.

Why Standard PT Isn’t Always Enough

Most PT programs focus on restoring daily function, not explosive, sport-specific performance. That leaves gaps when it’s time to actually get back on the field or court.

Common rehab “blind spots” we see:

  • Strength plateaus – Early gains are made, but hip and glute strength never reach the level needed for sport.
  • Stability issues – Core, pelvis, and hip control are rarely challenged in real movement patterns.
  • Power and speed training – Cutting, sprinting, and rotational demands aren’t reintroduced gradually.
  • Movement mechanics – Small compensations (like overusing your low back or quads) persist and eventually flare up.

What Advanced Labral Rehab Should Include

Getting back to sport requires going beyond pain-free daily activity and pushing toward true athletic readiness.

An extended, sport-specific program should cover:

  • Progressive hip and glute strength work – loading beyond bodyweight and isolating weaknesses
  • Core integration – controlling the pelvis under load and during rotation
  • Dynamic drills – single-leg hops, cutting, change of direction with progression
  • Movement re-training – analyzing and correcting faulty squat, lunge, sprint, and jump mechanics
  • Load management – knowing how much to push and when to back off to avoid flare-ups

A college soccer player came to our clinic 4 months after her labral repair. She had “graduated” from PT but still couldn’t sprint without hip pain. After a full movement screen, we found major weakness in her rotational core control and limited hip extension power.
